The work covers both compliance and advice. On the compliance side that typically means Self Assessment tax returns for clinicians with NHS, private or locum income, annual accounts and Corporation Tax returns for incorporated practices, VAT returns where relevant, payroll and pension auto-enrolment for practice staff, and bookkeeping to keep records accurate throughout the year. On the advisory side, healthcare clients usually want help with allowable expenses, business structure decisions, profit extraction and pension planning, and interpretation of how a particular income stream should be treated for tax.
It is not a legal requirement, but mixed and irregular income is where generalist support often falls short. Locum work, bank shifts and short-term contracts can involve expenses across multiple sites, travel between trusts or practices, and periods where income varies significantly. An accountant familiar with healthcare work will know which costs are typically claimable, how to keep records for irregular income, and how to manage payments on account so an unexpected tax bill does not derail your cash flow.
NHS salary is normally taxed through PAYE, and the figures already appear on your payslip or P60, so they are reported as employment income. Private fees, medicolegal work and self-employed locum income are usually reported separately as self-employment or miscellaneous income, with expenses claimed against them. The important part is making sure the two are not mixed up — claiming expenses against employment income that actually belong to self-employment, or vice versa, is a common error that can lead to queries from HMRC.
Costs that are wholly and exclusively for the purposes of the work are generally allowable. In practice these often include professional indemnity insurance, GMC, NMC, GDC or HCPC registration fees, CPD and training that maintains existing skills, clinical equipment and instruments, specialist clothing and laundry where required, professional subscriptions, and travel between work sites. Mileage for business journeys, a proportion of home costs where a room is used substantially for work, and accountancy fees are also commonly claimed. Each claim needs supporting records, and personal or commuting costs are not allowable.
If a practice employs anyone — receptionists, practice nurses, healthcare assistants, salaried clinicians or administrative staff — then PAYE, payslips, RTI submissions and pension auto-enrolment duties apply. Associates and locums add another layer, because their tax status depends on the terms of the agreement rather than the label used. Payroll support covers payslips, PAYE and National Insurance calculations, submissions to HMRC, auto-enrolment assessment and contributions, and payslip-format record keeping that keeps the practice compliant if HMRC or The Pensions Regulator asks questions.
Medical care provided by registered healthcare professionals is generally exempt from VAT, which is why many clinicians never register. However, mixed practices can find that some supplies are standard-rated — cosmetic treatments, retail sales of products, some medicolegal reports and certain non-clinical services are common examples. Once taxable turnover passes the registration threshold, or where exempt and taxable supplies are both made, partial exemption rules and input tax recovery need careful handling. Reviewing this early is usually cheaper than correcting it later.

Records can be kept through cloud accounting software fed by bank feeds, with receipts captured as you go, or through a simpler system if you prefer. Whichever route is used, the priority is that your records are complete and organised well before filing deadlines, so returns are prepared from accurate information rather than reconstructed at the last minute.
It begins with a conversation about how you work — whether you are employed, self-employed, in a partnership or running a limited company, what income streams you have, whether you employ staff, and what deadlines are coming up. From there, your existing records, returns and structure are reviewed to establish what needs doing and where improvements are possible. You then receive a clear outline of the support recommended, what information will be needed from you and how the working relationship will run, so you can decide with a full picture rather than a vague estimate.
