Healthcare clients often have income from several sources at once, expenses that are specific to clinical practice, and structures such as partnerships or limited companies set up around a practice rather than a trade. An accountant familiar with healthcare work already knows that indemnity cover, professional registration fees and locum arrangements behave differently from ordinary business costs, and can spot the issues before they become problems rather than asking you to explain them from scratch.
Two areas usually matter most. First, off-payroll working rules can apply where a public authority such as an NHS body engages your company, which affects how a contract should be structured and how tax is handled. Second, the way you take money out of the company — salary, dividends or a combination — has tax and national insurance consequences, and the best approach depends on your level of profit and other income. It is worth reviewing both areas before signing contracts rather than afterwards.
Not always, but healthcare professionals frequently do. If you have private fees, locum income, rental income, untaxed savings or investment income above the reporting threshold, or income from teaching, expert witness work or medico-legal reports, a return is usually required. The higher income child benefit charge can also bring people into Self Assessment. If you are unsure whether your circumstances trigger a filing requirement, it is worth checking rather than assuming, since the obligation rests with you.

Where a practice makes both exempt and standard-rated supplies, it is partially exempt, which affects how much input VAT can be reclaimed. Because the rules interact, it is usually worth reviewing your position before making registration decisions or changing what you offer.
Yes. Practice payroll commonly includes receptionists, nurses, administrators, associates and occasional locums, each of which can be treated differently for PAYE purposes. If you would prefer to meet, that can be arranged — but remote working means your location is not a barrier to getting specialist healthcare accounting support.
You should keep records that support every figure on your tax return — invoices, receipts, bank statements, mileage logs, contracts, payslips and dividend paperwork. HMRC generally expects records to be retained for at least six years after the relevant tax year, and longer in some circumstances. Digital record keeping makes this far more manageable, particularly when income arrives from multiple sources throughout the year.
